"BASIC** (1.5 HOURS) $300 Party includes guest of honor + their parents + 10 guests. Additional guests $10/guest. $50 non-refundable deposit due at booking. Includes set up and clean up. The party will consist of a mini dance class, dance games, glow party and cake/cupcake celebration. WE PROVIDE: Tablecloth. napkins, paper plates/cups. flatware, balloons, birthday banner, juice, water, and party favors. YOU PROVIDE: Cake, Candles, matches, cake knife and any additional snacks.

*DELUXE** (2.5 HOURS) $475 Party includes guest of honor + their parents + 10 guests. Additional guests $15/guest. $100 non-refundable deposit due at booking. Our Deluxe Birthday Party includes everything in the Basic package plus: a pizza party (cheese or pepperoni) and a craft for your guest to create and take home.

Policy/Agreement:

Your Birthday Party should be booked two weeks in advanced. Your booking is not complete until the non- refundable deposit stated above is received. The remaining balance of your party is due before the party start time. If you need to change the numberof guest/parents arriving, you have up to 48 hours to do so. After this time frame you will be charged for the number of guest/parents stated on this form. Guest of honor and guests have a 15-minute arrival time and departure time window. Set up and clean up is done on behalf of Danbury Dynamic Dance & Fitness LLC therefor, there is no reason to be earlier or later. I, (print full name),the undersigned, for myself and each sponsored dependent and guest, assume full responsibility for death, or any other injuries or damages which may occur to me or my sponsored guests or dependents in. on. or about the premises of the facility and do hereby fully and forever release and discharge Danbury Dynamic Dance & Fitness LLC from any and all suits, claims damages cost and expenses of every kind, in conjunction with the use of the facility and equipment. I. the undersigned, for myself and my sponsored dependents and guests further agree to useall related equipment. props and activity area properly and leave them in good condition. I. the undersigned. understand the activities which will be held during the party and have had my questions answered to my satisfaction. I, the undersigned,certify that the information I have given on my registration form is complete and accurate. By my signature below, I agree to the provisions of this Release of Liability Waiver for myself, for my sponsored dependents and guests, and for my heirs and assigns intending to be legally bound.
