  • Use this form to petition for an exception to university policy. Provide a clear statement that includes the specific exception you are seeking, and a justification that includes the conditions that make it appropriate to make this exception.  The the most common petitions are included on this form. If your request is not listed, use OTHER.  

    No petition is valid until approved and submitted.

    Do NOT use this form for the following: 

    • Reinstatement from Academic or Administrative Dismissal - Submit a Petition for Program/Course Reinstatement
    • Grade Dispute - Submit a Grade Grievance form (contact Registrar)
    • Formal complaint in response to actions by any member of the campus community - Submit a Student Grievance form (contact Office of Student Life)

  • Credit by Examination

    You must have completed a minimum of 12 graded units at UWest before requesting credit by examination.

    • Under REQUEST: provide course code you wish to test out of.
    • Under JUSTIFICATION: explain what qualifies you to take the exam.

    If approved, you must pay the exam fee (currently $100 per unit) prior to testing. This fee is due whether or not you pass the exam.

    Credit is granted if you receive a designated passing grade on the exam. This varies by exam and degree level. Credit granted will be posted to your transcript with a grade of PASS. 

    The maximum credit by examination allowed varies by degree level. Details are provided in catalog.

    Credit earned by examination does not apply toward the residence requirement for graduation.

    Approval: Advisor, Chair, CFO.

  • Late Add/Drop, Registration, Withdrawal

    Under REQUEST, provide specific exception you are seeking, including course code(s).

    Under JUSTIFICATION, provide valid reason for inability to withdraw or register within proper time period. A valid reason should indicate extenuating circumstance beyond the student's control.

    Please note: Not being aware of deadlines or having a failing grade after the drop deadline are not beyond the student’s control and therefore not valid reasons.

    You may be asked to forward validated documentation for illness or other reasons cited in your justification.

    If approved, you will need to submit required Registration/Add/Drop, or Withdrawal form. You must pay associated fees, if any.

    Approval: Advisor, Chair, CAO or CSSO, Financial Aid Officer.

  • Undergraduate Requesting Approval to Enroll in Graduate Course

    This option is open to students who have demonstrated academic ability and modeled exemplary behavior and maturity in the classroom and UWest community

    You must haved earned a minimum of 75 degree-applicable units with a 3.0 cum gpa and 3.5 cum gpa in major courses, and may apply no more than two courses (6 units) at the graduate level towards graduation.

    Under JUSTIFICATION: Provide explanation of why the exception to policy is appropriate.  

    Approval: Instructor, Student’s Department Chair, Course Chair if course is offered by another department.

  • Course Overload

    Approval is required if you are requesting units exceedingn the maximum allowed in fall or spring semester.

    • Undergraduates:     18 units allowed
    • Graduate students: 12 units allowed

    You must be in good standing to qualify for an overload.

    Under JUSTIFICATION, demonstrate how you will handle the additional workload without jeopardizing your success.

    If approved, you are subject to standard add/drop and withdrawal deadlines for all courses.

    Approval: Advisor, Chair, Financial Aid Officer.

  • Tutition Adjustment

    Use this form when requesting a reversal or adjustment of tuition for either of the following reasons:

    1. New student cancellation submitted after the first day of the term, but before the term census date
    2. Change in enrollment due to circumstances not in the student’s control, such as an error in advising

    Under REQUEST: explain the adjustment being requested.

    Under JUSTIFICATION: explain why the adjustment is warranted. 

    This form may be initiated by the university.

    For any financial adjustments other than an adjustment in tuition as described above, please see the Accounting Office. Do NOT use this form to request a refund, deferral, fee waiver, or payment plan.

    Approval:

    A: CFO, CSSO, CAO, Enrollment Manager, FAO.

    B: CFO, CSSO, CAO, FAO

  • Transfer Credit Preapproval

    Request preapproval prior to enrolling in courses at other institutions if you hope to transfer that credit to UWest. If you have not been preapproved, courses may not be eligible to transfer.

    All requests must meet UWest’s published transfer credit guidelines.

    If approved, credit will be granted upon receipt of official transcript showing appropriate passing grade:

    • Undergraduate = C (2.0)
    • Graduate = B (3.0)

    One form per college. You may list up to five courses on the request. If you would like additional courses reviewed with this submission, enter the first five courses on this form, and send an email to the registrar listing any additional courses. Be sure to include all required information. 

    Approval: Core curriculum: Program Chair, Registrar.
                   General education and elective curriculum: Registrar.
