  • Use this form to resume your studies following a break in enrollment if you have been out for less than two full consecutive terms of enrollment, excluding summer. You may return under the catalog in effect during your last term of enrollment unless you opt to change your major or concentration during the reentry process. 

    • Have you been out for two or more full terms? Submit an Application for Readmission to the Admissions Department. 
    • Are you requesting reinstatement immediately following a dismissal, with no break in enrollment? Submit a Reinstatement Petition.

    In addition to this form, submit a Registration form to select your classes. Contact your advisor to discuss your courses options.

    You will be notified once your reentry has been approved by Accounting, Financial Aid, and Academics, and if you are on an F-1 visa, the International Student Advisor. You may receive an email from any of these departments if additional information or action is required for approval.
