- OPTIONAL: Additional documents to share with the potential awardee (i.e. letters from the girls, drawings, parent letters, pictures, etc.)
How does this form work?
This form is designed to collect are nomination information from multiple individuals in a streamlined process. One individual will start the nomination and then additional contributions can be completed.
- The first nominator will start the nomination form with the nominee's information, their personal nomination, and the contact information of additional contributors.
- Once submitted, two actions will occur:
- The initial nominator will receive a confirmation email with a link to edit the form. The link can be copied and shared with additional nominators.
- Anyone listed as an additional nominator in the initial submission will receive an email to update the nomination form with an edit link.
- Additional nominators will follow the link and add their own details to the nomination. Be sure to not overwrite someone else's submission,
- All nominators have until February 14 to add, edit, and complete the nomination.
