RELEASE AND INDEMNITY AGREEMENT
As part of being a member, or a temporary participant of New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball activities and for using the associated practice times and playing spaces, facilities and equipment, and in consideration of the voluntary nature of such participation and use, I hereby release, hold harmless, and forever discharge the New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball its volunteers, agents and other members of New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball from any and all liability claims, demands, actions, and causes of actions whatsoever arising out or related to any loss, property damage, or personal injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball its members or agents, or otherwise, while participating in such activity. Such participation includes practices, clinics, club functions, competition, and travel to and from all tournament activities.
I am fully aware of the risks and hazards associated with participation and use of the practice space, facilities and equipment. I hereby elect voluntarily to participate in said activities and fully acknowledge that the full responsibility for any risk or loss, property damage, or any loss or any personal injury, including death, that may be sustained by me or any loss or damage to property owned by me as a result of being engaged in such activities, whether caused by negligence of the New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball its volunteers, members or agents, or otherwise. I further acknowledge that I have procured on my own adequate insurance for such loss, damage, and injury. I further agree to indemnify and hold harmless the New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball its volunteers, members, agents, from any loss, liability, damage or cost, including court costs and attomey's fees that may incur due to my participation in said activities, whether caused by the negligence of the New York City Team Handball Club Inc., NYC Department of Education, El Faro Beacon Program at River East Elementary, St. Nicks Alliance Beacon Program at John Ericsson School and USA Team Handball its volunteers, members, agents, or otherwise.
This release and hold harmless agreement is binding on myself, my heirs, assigns, and personal representatives.
