    Congratulations on your decision to participate in an exercise program! With the help of your Personal trainer, you greatly improve your ability to accomplish your training goals faster, safer, And with maximum benefits. The details of these training sessions can be used for a lifetime. In order to maximize progress, it will be necessary for you to follow program guidelines during Supervised and (if applicable)unsupervised training days. Remember, exercise and healthy Eating are EQUALLYimportant! During your exercise program, every effort will be made to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program. By signing below, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility isassumed by the leader of the program. ___________________________________________Personal Training Terms and Conditions ___________________________________________ 1.Personal training sessions that are missed will result in forfeiture of thesession and a loss of the financial investment at the rate of one session. Youare advised to make up for that session using one of your programs when you are able to. Unless other arrangements have been made with the trainer. ___________________________________________ 2.Clients arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer. ___________________________________________ 3.The expiration policy requires completion of all personal training sessions within the agreed upon time from the date of the contract unless other arrangements have been made by the Personal Trainer. ___________________________________________ 4.No personal training refunds will be issued for any reason, including but not limited to relocation, and unused sessions. ___________________________________________ 5.If I, the Personal Trainer miss a session. Your sessions will resume when I am able to return. ___________________________________________ 6.The personal training sessions are either paid on a month to month basis or paid in full. If the 6 month plan is chosen and you cancel early you will owe the difference you have saved in this option which is $100 extra per month you partook in personal training sessions. After the 6 months of you have chosen this option you are able to put an end to the sessions by texting me the personal trainer and letting me know before the next debit date and only paying 1 more debit with receiving 1 more month of personal training. If you are on a month to month plan then the way to end the sessions are the same you can just text me whenever you would like to end the sessions. ___________________________________________ 7.I give permission for the personal trainer to promote his business with before and after photos or messages to help promote his business, with the condition that he asks for my permission first each time before doing so. I understand that taking progress photos will also help me to further notice the results of my training. ___________________________________________ 8.Nutrition coaching happens after you have downloaded the app my fitness pal, added the personal trainer on the app and have kept track of everything you have consumed for at least 7 days. If you do not do this then the nutritioncoaching will just be regular advice given in person based on healthy eating according to the Australian dietary guidelines. I WISH YOU THE BEST OF LUCK ON YOUR NEW PERSONAL TRAINING PROGRAM!
