Preamble
This agreement is predicated on the date as set above, between MyHB Center and {name}, hereinafter referred to as client. Membership is for the purpose of providing the client with the utmost flexibility and convenience offered to members of the MyHB Center.

Client hereby agrees to join the membership and accepts the terms and conditions set forth below:

Membership Fee
The annual membership fee is $50.

Membership must be renewed on an annual basis. A renewal reminder will be emailed or texted at least two months in advance of the renewal date.

Membership Age
New members must be at least 18 years old. Under the age of 18 years, parental consent is required to join.

Membership Benefits
* Online is optimized for use with the Google Chrome browser
* Higher membership level offers greater cost saving for treatments
* The latest non-invasive tools and technology are utilized
* Unrestricted options in selecting treatment types
* Sensitive information protected by a secure network coupled with a HIPAA-compliant environment
* Online scheduling of treatment date
* Online payments are by credit card, debit card or cash app
* Ability to integrate, fat reduction, muscle sculpting, vaginal rejuvenation, skin revitalization and hair removal as an overall treatments plan
* Numerous others

Membership Level
This agreement entitles the Client to receive unlimited and flexible services based upon the membership level purchased. The four levels are identified as bronze, silver, gold and diamond.

The purchase of a level establishes the number of points received to apply for treatments. Please complete Exhibit A below.

Customer Service
* Services will be rendered by MyHB Center upon receipt of the online request 
* Client agrees to use reasonable judgement in determining their treatment needs
* Client's request made online for treatments will require MyHBC's approval
* MyHB Center hereby agrees to accommodate Client to the best of its ability in adhering to the Client's selected appointments and service requests

Restrictions
The Client agrees that he/she will not:
* Share their login information with other members or non-members
* Reproduce any information received as part of their membership and share it with non-members
* Post inappropriate, inaccurate or offensive content or images on social media platforms or internet forums
* Use the online service in a compromising manner which exposese the system to  attacks by viruses, hackers or unauthorized users

Termination
MyHB Center has a right to terminate the Client's membership if any of the terms and conditions of the Agreement have been violated. The Client may terminate their membership at any point or any reason. Client will not be refunded unused funds or points that remain in his/her account

Privacy
MyHB Center will not share any contact or personal information about a Member with other members, non-members or any third parties. Information of a member will be handled and managed by MyHBC in a confidential manner.

Guest Preamble
This agreement is predicated on the date as set above, between MyHB Center and {name}, hereinafter referred to as client. Guest is for the purpose of MyHB Center to provide service to non-member clients.

Clients, who are interested in enhancing their appearance within a specific timeframe and within a limited time period, may elect aesthetic treatments as a Guest. 

Such circumstance as special events, weddings, vacations or milestone celebrations could be some of the factors to consider treatments as a Guest. 

Guest Fee
No registration fee is required and Guest deemed to be a non-member.

Guest Age
Guest must be at least 18 years old. Under the age of 18 years, parental consent is required to join.

Limited-Time Treatment
Guest for treatments is considered to be an ad-hoc service without specific long-term and customized planning. The focus is on delivering expedient service with safe, effective results under condensed timeline, time constraints or special occasions.
