Do you qualify for the $250 perfect attendance stipend?

The perfect attendance stipend is paid on:

- The first mid-month payday after Winter break.

- The first mid-month payday after the campus closes for Summer break.

The start and stop dates on this form must coincide with the Fall & Spring semesters' first and final workday in order to be considered.

I also understand that should I receive payment and am found to have been absent, the stipend will be revoked in one lump-sum payment from the check immediately following the discovery.
