General Policies and Waiver:
1. Parent(s) of kids under 5 years old must remain within the facility during the session.
2. To waive all claims that I have or may have in the future against B-Ballerz Athletic Academy, its members, employees, volunteers, and independent contractors (all of whom are hereinafter collectively referred to as “B-Ballerz Athletic Academy”).
3. To release B-Ballerz Athletic Academy from any and all liability for any loss, damage, injury, or expense that I may suffer, or that my next of kin may suffer as a result of my participation in the activity of basketball skills training, due to any cause whatsoever including negligence, breach of contract, or breach of a statutory or other duty of care.
4. Please ensure all children's belongings are clearly labelled with their name.
5. To hold harmless and indemnify B-Ballerz Athletic Academy from any and all liability for any damage to the property of, or personal injury to, any third party resulting from my participation in a basketball training session.
6. This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns, and representatives in the event of my death or incapacity.
7. I, the undersigned, hereby authorize B-Ballerz Athletic Academy to photograph, take motion pictures of, take video footage, and/or make electronic sound recordings. I understand that these pictures and/or videos may be posted on B-Ballerz Athletic Academy’s social media accounts including Facebook, Instagram, and other accounts.
8. In order to cancel or temporarly suspend your membership, please provide two weeks notice.
9. In entering into this Agreement, I am not relying upon any other oral or written representations or statements made by B-Ballerz Athletic Academy other than what is set forth in this Agreement.
10. Make up sessions can be booked for missed classes. (Missed sessions have no cash value).
Pausing, Cancelling Your Membership, Overdue Accounts, Make up Sessions Policies:
1. Members may request to pause their membership at any time, but note that when resuming, there may be a price change based on current rates.
2. Pausing your account does not guarantee your spot in any class or program upon resuming. Availability is subject to current capacity.
3. You must provide at least 2 weeks’ notice before your next scheduled payment to request a pause. If a request is made less than 2 weeks before the payment date, a $20 administrative fee will be charged for processing a refund.
4. If you wish to cancel your membership, a 2-week notice is required before your next payment date to avoid being charged for the upcoming cycle.
5. Cancellations requested with less than 2 weeks’ notice will incur a $20 administrative fee to process the refund.
6. If your payment is overdue by more than 10 days and the automatic payment cannot go through, your account will be paused and your membership cancelled.
7. While the account is paused due to overdue payment, you will be unable to participate in any weekly classes or programs.
8. Your membership will remain on pause until the overdue balance is paid in full.
9. Members are welcome to schedule make-up sessions for any missed classes during their active membership period. However, please note that once a membership is canceled, all remaining make-up sessions will be forfeited and cannot be redeemed. We encourage members to utilize their make-up sessions while their accounts remain active.
