  • HART Advisor Contact Information

    Your HART advisor should be:

    • A school staff member (teacher, guidance counselor, athletic coach, child nutrition staff, etc.) who is interested in promoting healthy eating
    • Able to meet with you on a monthly basis to discuss and support your upcoming nutrition initiatives
    • Willing to connect you with other school staff and the administration

  • Texas Department of Agriculture (TDA) Photography/Video release, Waiver Consent

  • I understand that the activities of Health Ambassadors for a Ready Texas (HART) will include, without limitation, emailing, phone calls, webinars, in-person meetings, and other forms of online communication necessary to stay in contact with student ambassadors, school staff, TDA staff and others. I further understand that HART Student activities may involve excursions that may involve vehicular transportation or walking, which may involve risks inherent to such activities including but not limited to vehicle accidents, physical exertion, falls, burns, cuts and contact with other participants. I hereby acknowledge these risks and assume all risks, including personal injury and fatality, arising out of my child’s participation in HART and the accompanying activities. I attest that my child, or myself, if I am 18 years of age or older, (Participant), is able to participate in HART. I acknowledge that the Participant has been told to discontinue and request support if he/she experiences any condition that would make participation in the program activities difficult or unsafe to continue. I agree for myself, my heirs, executors, and administrators, not to sue and to release, indemnify, defend and hold harmless the Texas Department of Agriculture, their affiliates, officers, directors, employees, agents, and all sponsoring businesses and organizations and their agents and employees, from and against any and all liability, claims, demands and causes of action whatsoever, arising out of or brought in connection with my child’s participation in this event and related activities – whether resulting from the negligence of any of the above or from another cause. I affirm that all information provided in the HART Application is true and accurate to the best of my knowledge, and understand that any deliberate falsifications, misrepresentations, or omissions of fact may be grounds for rejection of my application. TDA reserves the right to accept or reject the application as it sees fit. I voluntarily consent and authorize TDA to contact and obtain information from any listed references, or any persons or organizations who may have knowledge relevant to my application. I, the undersigned, hereby grant permission to TDA and its employees, officers, agents, representatives and assigns (TDA) to use photographic and video images taken of me and/or my child(ren). I grant permission together with the right to use, publish, copyright and reproduce in whole or in part any such images in agency publications such as newsletters and magazines, and to use such images in electronic versions on agency websites or other electronic form or media for any legitimate purpose of TDA. I hereby waive any right to inspect or approve the finished photographic or video images or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph. I hereby voluntarily release and hold harmless TDA from any and all liability arising out of or in any way related to the use of such images, video footage and comments, including but not limited to any liability arising by virtue of any blurring, distortion, alteration, illusion, editing, or use in composite form, whether intentional or otherwise, that may occur in the making or processing of the finished product. By signing below, I hereby grant TDA permission to use, publish, and share publicly the Participant’s first and last name. This Release, Waiver and Consent form shall be broad and inclusive as is permitted by law. If any portion of it is held invalid, the balance shall continue in full force and effect. I am the legal guardian of the HART Student, the Participant, and I hereby consent to his/her participation. I have read and explained the preceding Release, Waiver and Consent form to my child, and I hereby agree to its terms on behalf of myself and the Participant.
