Volunteer Waiver and Release Form

In return for being allowed to participate in Freedom International Christian Training Center VBS and all related activities, including any activities incidental to such participation (“Volunteer Activities”), the under- signed Volunteer releases and agrees not to sue Freedom International Christian Training Center or its officers, directors, employees, sub-contractors, sponsors, agents and affiliates from all present and future claims that may be made by me, my family, estate, heirs, or assigns for property dam- age, personal injury, or wrongful death arising as a result of my participation in the Volunteer Activities wherever, whenever, or however the same may occur.

I understand and agree that the Church is not responsible for any injury or property damage arising out of the VBS Volunteer Activities, even if caused by their ordinary negligence or otherwise.

I understand that participation in the VBS Volunteer Activities involves certain risks, including, but not limited to, serious injury and death. I am voluntarily participating in the VBS Volunteer Activities with knowledge of the danger involved and I agree to accept all risks of participation.

I also agree to indemnify and hold harmless Freedom International Christian Training Center for all claims arising out of my participation in the VBS Volunteer Activities.

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state in which the VBS Volunteer Activities take place and agree that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.

I also acknowledge that Freedom International Christian Training Center has not arranged and does not carry any insurance of any kind for my benefit or that of Volunteer, my parents, guardians, trustees, heirs, executors, administrators, successors and assigns. I represent that, to my knowledge, I am in good health and suffer no physical impairment that would or should prevent my participation in VBS Volunteer Activities.

I also understand that this document is a contract which grants certain rights to and eliminates the liability of Freedom International Christian Training Center.

 

In return for being allowed to participate in Freedom International Christian Training Center VBS and all related activities, including any activities incidental to such participation (“Volunteer Activities”), the undersigned Volunteer or Parent/Legal Guardian of Volunteer if Volunteer is under age 18 (hereafter referred to using “I”, “me”, or “my”) hereby grants Freedom International Christian Training Center, and each of its subsidiaries, affiliates, agents, advertising or promotional agencies, and partners, and all such entities’ officers, directors, agents, employees, respective successors and assigns (collectively, “Authorized Parties”), the absolute and irrevocable right and permission to use, publish, broadcast and/or copyright the use of Volunteer’s name, address, voice, photograph and/or likeness, caricature, and personal information, in its current form or as retouched, digitized, cropped, altered, distorted or modified in any way, in any and all advertising, promotional, or other materials based upon or derived from the Volunteer Activities in any manner, in any media whatsoever for any and all purposes, including by way of example but without limitation advertising, promoting or publicizing products and services throughout the universe, in perpetuity, in any and all media now known or hereafter devised (including without limitation on the Internet), without additional compensation.   I further agree that anything derived therefrom will be owned solely by the Authorized Parties. I shall not authorize the use of any print, negative or other copy thereof by anyone other than the Authorized Parties.

Freedom International Christian Training Center is committed to the protection of our children, youth, and vulnerable populations, and has established its Child Protection Plan (CPP) to provide for a safe, secure environment for minors who participate in our programs. We are equally committed to protecting the Church, our staff members, and our volunteers from false or unwarranted accusations.

I acknowledge that I have received a copy of the Church’s Child Protection Plan (CPP) which describes important information about the Church’s policies, procedures, and guidelines regarding child safety and protection.

 
I acknowledge my understanding and agreement that as a Church employee or volunteer, I am mandatorily required by Church policy to report any known or reasonably suspected child abuse, even if I am not a statutorily mandated reporter as defined by California Penal Code §11164-11174.3.

 
I acknowledge that, since the information, policies, and procedures in the CPP are subject to change, revisions to this manual may occur, and the Church shall have sole authority to add, delete or adopt revisions to its Child Protection Policies, with or without advance notice. 

 
I understand and agree that I will read and comply with the policies and procedures contained in this Child Protection Plan as well as any subsequent revisions, that I am bound by the provisions contained herein, and that my continued employment or volunteer service is contingent, in part, upon my adherence to this Child Protection Plan. 

I understand that I should consult with Human Resources and/or the childrens ministry leaders if I have any questions.  

In the event that I become incapacitated during VBS Shout 2026, I hereby authorize Freedom International Christian Training Center to consent to X-ray, examination, anesthetic, medical, dental, or surgical diagnosis, treatment, and/or hospital care deemed necessary or advisable by a licensed physician. It is understood that this authorization is given in advance of any specific diagnosis, treatment, or hospital care being required. It is given to provide authority and power on the part of Freedom International Christian Training Center to give specific consent to the diagnosis, treatment, and/or hospital care, which, in the best judgment of a licensed physician, is deemed necessary.

I authorize the use of first aid by Freedom International Christian Training Center if Freedom International Christian Training Center deems it is needed.

I authorize the use of CPR if Freedom International Christian Training Center deems it is needed.

I authorize Freedom International Christian Training Center to secure emergency medical care and transport if deemed necessary by Freedom International Christian Training Center.

I agree to assume all costs of emergency care and transportation.

To the best of my knowledge, I have listed below all of my allergies, including food and medical allergies, medications being taken, medical problems, and other pertinent information.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A CONTRACT, AND I SIGN IT OF MY OWN FREE WILL.

CHILD PROTECTION PLAN ACKNOWLEDGEMENT

Medical Release Waiver

I authorize Freedom International Christian Training Center to administer basic first aid if it is deemed necessary.

I authorize the use of CPR if it is deemed necessary.

I authorize Freedom International Christian Training Center to secure emergency medical care and transportation if deemed necessary.

I agree to assume all costs associated with emergency medical care and transportation.

To the best of my knowledge, I have listed below all allergies (including food and medical allergies), medications currently being taken, medical conditions, and any other pertinent health information.

Certification

I certify that I have read this document and fully understand its contents. I acknowledge that this is a legal agreement, and I sign it voluntarily and of my own free will.

As a Group Leader or Volunteer attending VBS Shout 2026, I will:

Keep Jesus at the center.
Leading is a way for others to see Jesus in me. I’ll keep Jesus at the center of VBS Shout 2026 by making sure everything I do honors Him.

Respect others.
I can glorify God through how I treat people. I’ll show respect to everyone around me, including staff, other leaders, volunteers, and youth through both my words and my actions.

Be a good steward.
I don’t want to take anything for granted. I’ll take care of all facilities, vehicles, and property. I will leave every location better than I found it.

Set an example.
Children are watching me. I’ll be a positive role model for others to follow.

Bring the energy.
I want youth to have a positive experience. I’ll do my best to bring energy and fun to create an environment that helps youth grow closer to Jesus.

Be proactive.
Children need clear guidelines (see the rules below). I’ll set clear expectations for my group so they can stay safe.

Engage.
VBS Shout 2026 is about our children. I’ll make sure they have my attention and interest by listening, responding, and limiting distractions (e.g., texting, talking with other leaders during activities, scrolling social media, etc.).

Children Rules
Leaders and Volunteers, please be aware that these are the rules children are expected to follow:

Listen:
Children must obey leaders and staff at all times.

Bullying:
Bullying will not be tolerated. This includes any attempt to harm, intimidate, isolate, or pressure someone to do something.

Substances:
No drugs, tobacco, vapes, e-cigarettes, or alcohol. Bags may be checked at initial check-in.
If a child takes prescription medication, it must be packed in a clear bag with a doctor’s note explaining why the medication is needed.

Clothing:
No inappropriate clothing. This includes clothing with logos, phrases, or graphics that contain profanity or sexual/offensive content. Revealing shorts, shirts, or swimsuits are not allowed.

Weapons:
No weapons are permitted, including pocket knives.

Contact:
No inappropriate displays of affection with another child. This includes, but is not limited to, kissing or any kind of sexual behavior.

Behavior:
Stealing, lying, fighting, and other disruptive behavior will not be tolerated.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
