本人明白在下方簽署後，將表示本人已完全明白並願意承擔在此活動期間所涉及相關的自然和醫療風險及責任。本人亦明白本人必須遵守有關活動的安全守則，以及遵守是次活動教練的指示。在下方簽署後，將代表本人同意上述的一切條款，以及聲明本人身體狀況良好．適合參與下列活動．並無任何傷患或不適影響本人進行訓練。本人豁免及放開追究毅行教室、毅行教室員工及毅行教室之義工。本人同意並瞭解，若有任何個人財產在教室會址範圍內或其他場所丟失或被盜，毅行教室概不負責。
本人明白在下方簽署後，將表示本人（參加者／參加者家長／參加者監護人）同意TTR採用本人參與上述計劃所拍攝的照片／影片，作內部的檢視訓練進度、教育培訓及呈交計劃報告之用；本人同意TTR採 用本人參 與上 述 計 劃 所 拍 攝 的 照 片 ／ 影 片，作 對 外 宣 傳，包 括刊 物、年 報 、 網 站 、 傳 媒 介 紹 等 之 用 ；本人同意如 本人接受由其他機構安排的傳媒訪問，如訪問內容與上述計劃有關，TTR會事先取得 本人同意，本人需要事先取得 TTR的同意。
By signing below, I acknowledge that I fully understand and am willing to assume the natural and medical risks and responsibilities involved in this activity. I also understand that I/participant must follow the safety guidelines for the activity and follow the instructions of the coach of this activity. By signing below, I agree to all of the above terms and declare that I/ participant is in good physical condition, suitable for participating. in the above activity(s) and has no injuries or discomforts that would affect my/ the participant's training. I waive and release any claims against TTR Trailwalker Teaching Room Charity Foundation (“TTR”), its employees, and volunteers. I agree and understand that if any personal property is lost or stolen within the classroom premises or elsewhere, TTR will not be responsible.
I, the undersigned (participant/parents of the participant /guardian of the participant), hereby agree to the use of photographs/videos taken of my child/ward participating in the aforementioned programme by TTR for the purposes of internal review of training progress, educational training, and submission of programme reports. I hereby agree to the use of photographs/videos taken of my child/ward participating in the aforementioned programme by TTR for external publicity, including but not limited to publications, annual reports, websites, and media introductions. I hereby agree that if my child/ward or I accept media interviews arranged by other organizations, and if the interview content is related to the programme, TTR will obtain my consent in advance, and I will obtain the consent by TTR in advance.
