Student Activity Waiver

I voluntarily assume all risks of loss, damage, illness or injury which I may sustain while participating in this activity. I acknowledge that there are risks associated with an activity of this kind, I hereby assume all risks of participating in this activity.

Further, I will refrain from instituting, processing, or in any way aiding any claim, demand, action or course of action for damages, costs, expenses, or compensation against College of Southern Maryland and its officers, agents and employees for, on account of, or hereafter growing out of, any injury or loss which may occur during the event described above.

Additionally, I release College of Southern Maryland and its officers, agents and employees from any and all claims, demands, and causes of action resulting from any injury or loss why occur during my participation in the event described above, whether arising through the negligence, omission, default or other action of any person or organization associated with the event described above.

Travel Regulations:

•I understand that I am traveling for the University on state business and shall conduct myself appropriately. All University and Student Code of Conduct policies are in effect, and I am expected to represent myself in an appropriate manner, abide by campus policies, and understand that I will be held accountable for my behavior.        

•In case of medical emergencies, Emergency Medical Services will be contacted. I authorize the College of Southern Maryland to secure emergency medical services for me. I will have my personal insurance information with me.

•In case of emergency, the following person should be contacted. (cannot be a person traveling/ participating in this trip)
