Introduction: This form is for Match Alteration Requests for the 2025 season. All requests require the details of the match as currently scheduled and the requested details for the match.

Requests must be made in accordance with the 'Rescheduling of Matches' within the relevant 2025 Competition Regulations. Please note that the final decision on your request is made by CF. This decision is final and not subject to appeal.
