Patients have told us how helpful it can be to connect with other people living with similar health conditions and who may have similar questions/concerns about making sustainable lifestyle changes. Patients have also shared how beneficial it has been to speak with others who are going through wellness journeys of their own. A Shared Medical Appointment (SMA) is an inclusive group visit in which participants can learn from specialists (and each other!) while also receiving an individual consultation with a doctor or physician assistant.
We will be using the Full Plate Living Core Program - a delicious, research-backed approach that can lead to more energy, lower cholesterol, better blood sugar, and weight loss:

- Module 4 – Getting Social, Finding Support, and Navigating Family Meals
- Module 5 – Meal Prep & Dining Out
- Module 6 – Snacking, Satisfaction & Emotional Eating
- Module 7 – Setting Yourself Up For a Lifetime of Health
- Module 8 – Reimagining Weight Loss
Participants may join at any point but it is strongly encouraged (and may be mandatory for some per our weight loss program requirements) to attend at least once a month for the best results. SMAs are biweekly, typically last 60-90 minutes, and are available in-person on Tuesdays (5:30-7PM) or virtually on Thursdays (12-1PM). If you have been diagnosed with obesity or a cardiovascular disease risk factor such as hypertension (high blood pressure), hyperlipidemia (high cholesterol), diabetes, or metabolic syndrome, your insurance may cover 100% of the costs for the SMA.
