I hereby grant Justice Necessary, its agents and licensees, to photograph, record or otherwise capture my or my minor child’s name, image, voice, and/or likeness in photographs or videos taken at the PeriodPalooza event.  I further irrevocably grant to Justice Necessary, its agents and licensees, the unrestricted right and permission, in respect of the photographs and/or video taken of me or my minor child, to use, re-use, publish or republish photographic images and video clips that in any way include myself and/or any minor child listed below.

I understand that I and/or the named minor child may be included in part or in whole in any and all
media for illustration, promotion, art, editorial, trade, website, video, social media, advertising or any other purpose whatsoever. I also consent to the use of any printed or electronic material
referencing me or my minor child. I understand that I waive any right to review or approve the
finished product(s) and that no compensation will be paid.

I hereby release Justice Necessary from any claims I may have now or in the future regarding the use of my or my minor child’s name, image, voice and/or likeness, including without limitation, any claims for compensation.  This release is binding on me, my heirs, assigns, and estate.

I am 18 years old or older, and/or I am the parent or legal guardian of the minor child listed on the application.
