    Use this form if you are terminating all utilities. If you are only terminating internet, TV, or phone service, please call us 801-804-4503.
  • If you are a homeowner, you may fill out this form anytime after both the buyer and the seller have signed closing documents.

  • If you are a commercial building owner, you may fill out this form anytime after both the buyer and the seller have signed closing documents.

  • We have found that projected closing dates often change. Therefore, you must wait until both the buyer and the seller have signed closing documents to terminate utilities. If you haven't yet signed closing documents, please do not fill out this form. 

  • If you are a tenant, we will terminate services the day that you choose on this form. Please note that we cannot backdate terminations.

  • Spanish Fork City bills for utilities the month after you use the service. Your final bill will be pro-rated through your termination date and will be due the month after termination (see table below).

  • If you paid a deposit, it will be applied to your final bill. If you are enrolled in ACH autopay, that will remain active for your final bill.

  • Please return equipment to the SFCN office at 657 West Center Street within two business days of termination. Equipment not returned will be billed to your account.
