2026 Regional Club & Association Affiliation Application

APPLICATION FOR 2026 CLUB & ASSOCIATION AFFILIATION

Volleyball South Australia welcomes applications for 2026 Affiliation. Please ensure you have read and understood the Volleyball SA Affiliation Brochure for 2026 and have the following information on hand to assist in completing this application.

1) Club/Association information (email, address, ABN, etc)

2) Club/Association Committee contacts

3) Latest Annual Report, Constitution, Incorporation Certificate and logo (file size cannot be bigger than 1024kb. If larger, please email to sophie.h@volleyballsa.com.au separately).

4) Relevant Child Safe Environment details and declerations.

Once you've started, please progress to the end of the final page and click SUBMIT in order to retain all of the completed information.

The affiliation period is 1 January 2026 – 31 December 2026.

Affiliation Fees (All fees inclusive of GST):

Gold (Regional) : $276

Affiliation agreement and payment:

Once the application has been completed with all relevant information, VSA will issue the affiliation agreement to the president. Please review, sign, and return the document to VSA within 14 days.

An invoice for the affiliation fee will also be sent to your association, with payment due within 14 days of receipt. The affiliation will commence once the payment and agreement has been received.

PART FOUR: CHILD SAFE ENVIRONMENT DETAILS AND DECLARATIONS
CHILD SAFETY COMPLIANCE DECLARATION

By this declaration, I (on behalf of the club/association) hereby acknowledge that I have read the current VA Member Protection Policy, adopted by VSA.

We confirm that we thoroughly understand the obligations required of our club/association by the:
- Child Safety (Prohibited Persons) Act 2016 (the Prohibited Persons Act)
- Children's Protection Law Reform (Transitional Arrangements and Related Amendments) Act 2017 (the Transition Act)
- Child Safety (Prohibited Persons) Regulations 2019
We agree to adopt and implement the Member Protection and Child Safety Policies adopted and implemented by Volleyball SA, throughout our club/association.

We understand that the Policies and Procedures Volleyball SA have in place are by no means a guaranteed action of prevention and understand that it is our responsibility as a club or association, to create and maintain the safest environment possible for our members.

We have read the suggested interview questions in the Child Safeguarding Policy and acknowledge VSA's recommendation to implement these questions as part of our staff and volunteer screening process.

Furthermore, our staff and volunteers understand their obligation to notify the Child Abuse Report Line (CARL) on 13 14 78 as soon as practical if they have any reasonable suspicion or concern that a child has been, or is being abused or neglected by an adult.

APPLICATION FOR AFFILATION

On behalf of the Club/Association, I wish to apply for the Affiliation with the South Australian Volleyball Association Inc (VSA). In applying for affiliation with VSA, the applicant organisation understands and accepts that payment of the base affiliation fee is required with this application.

The applicant organisation understands that by applying for affiliation with VSA the above named organisation must ensure compliance with all state and federal legislation and VSA affiliate related policies, including but not restricted to, member protection, sponsorship, work and safety and child safety policies and provide evidence of such as requested.

The applicant organisation understands that as it relates to the volleyball activities subject to this Affiliation agreement, they are bound under the Constitution, Regulations, and affiliate related policies of VSA and that VSA is under no obligation to continue this agreement should the organisation fail to uphold the terms within this agreement.
