Liability Release and Waiver Form

By completing this Liability Release and Waiver Form for HAVEN Missions, you acknowledge that some activities may involve certain risks and agree to participate willingly. You also confirm that you understand these risks and release HAVEN Missions, its leaders, volunteers, and partners from responsibility for injuries or losses that may occur during related events or activities.

I am at least 18 years old. I have voluntarily chosen to participate in the stated activity or trip offered by HAVEN Missions.

Key Risks Acknowledged
I understand that this Activity or Trip, like any other activity or trip, involves potential hazards or risks. These could lead to my illness, serious injury, or even death. I fully recognize and accept these risks.

My Release of Liability
In exchange for being allowed to participate, I accept all risks to my health, injury, or death from this Activity or Trip. I release HAVEN Missions—along with its board, officers, employees, and representatives—from any liability. This includes claims from me, my family, estate, heirs, or assigns for property damage, injury, or death. The release applies even if caused by their negligence or other factors.

My Promise to Indemnify
I also agree to protect (indemnify) and hold harmless HAVEN Missions, its board, officers, employees, and representatives. I will cover any liability for injury, death, or property damage caused by my own negligent or intentional actions (or failure to act) during the Activity or Trip.

Full Understanding
I have carefully read and understand this entire agreement. It serves as a complete release of all claims for my injury, death, or property damage during the Activity or Trip. It also requires me to cover liabilities from my actions.
