    Donation Amount:
    $550.00 - covers the cost of the tree, installation, and maintenance. Includes 1 replacement tree within 5 years of being planted.

    Location Requested: Commemorative trees are to be installed in pre-approved locations. If required, St. Clair Township has the right to remove and relocate a commemorative tree. Please select the location you would like the tree to be planted.

  • Please complete this form by clicking “Submit” at the bottom of the page. Once submitted, the Commemorative Program Application Form will be automatically forwarded to St. Clair Township’s Community Services Department for review.

    After the form has been reviewed and approved, we will contact you regarding payment.
