Please note: The Ellen Jane Smith Approach is a registered nonprofit corporation, and our application for federal 501(c)(3) tax-exempt status is currently pending. Participant Waiver & Release of Liability. By registering for and participating in EJ’s Parade of Hope, I acknowledge that participation in this community walk event is voluntary. I understand that outdoor activities and physical events may involve certain risks, including but not limited to slips, falls, weather conditions, interactions with other participants, and other unforeseen incidents.I certify that I am physically able to participate in this event, and if I am registering a minor child or other individual for whom I am legally responsible, I certify that they are also physically able to safely participate in this event.I acknowledge that I am responsible for the supervision, safety, and well-being of any minor or participant I register.I assume full responsibility for any risks, injuries, damages, or losses that may occur to myself or to the minor(s) or individuals under my care as a result of participation in this event.I hereby release, waive, and discharge The Ellen Jane Smith Approach, its directors, officers, staff, volunteers, event organizers, sponsors, partners, and affiliates from any and all liability, claims, demands, damages, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur during or as a result of participation in this event.I authorize event staff or volunteers to seek emergency medical assistance if necessary for myself or for any minor participant under my care, and I understand that I am responsible for any medical costs that may arise.I also grant permission to The Ellen Jane Smith Approach to use photographs or video taken during the event for promotional, marketing, or educational purposes.By completing this registration, I confirm that I have read, understood, and agree to the terms of this waiver and release of liability.
