In connection with the registration of your child's camp participation to be conducted by The Community Theatre of Greensboro (CTG), Guilford County Schools (GCS) and in consideration of your enrollment and/or consent of said child in such camps and/or performances, I the undersigned parent/guardian(s), acknowledge that I have been informed
on the activities to be offered to said child and consent thereto; represent that said child/children are in good health and physically fit and capable of participation in the theatrical camps and performances to be offered by The Community Theatre of Greensboro (CTG) and Guilford County Schools (GCS); acknowledge the risks and hazards of physical injury inherent in theatrical camps and performances and hereby assume all such risks and hazards; and I hereby release, waive, and agree not to assert against Community Theatre of Greensboro (CTG) and Guilford County Schools (GCS) , each of its directors, teachers, officers, supervisors, agents, or employees, any claim for injury to said child in consequence of or incident to such training, performance or other activities.

Week 1: June 22nd- 26thWeek 2: July 6th - 10thWeek 3: July 27th - 31stWeek 4: August 3rd - 7th
Payment of $25 registration fee and first weekly payment of $110 is due at the time of registration. Please also select the additional weeks that your student will be attending.

Please note that we can not prorate any weeks that children will not be in attendence all 5 days.
