Manual Payments:

• SCHOOL YEAR monthly tuition before the 25th of the month prior to care. eg: September tuition due before August 25th
• SUMMER weekly tuition 3 weeks before the affected week starts.

Recurring/Automatic Payments (via TuitionExpress)
I understand & agree to personal responsibility for payment not processed for any reason, and authorize childcare tuition charge to my provided card or account on a recurring & ongoing basis via ProCare's Tuition Express. My financial institution honoring these charges constitutes receipt of payment, and:

• Summer charges are weekly, 3 weeks in advance of care week. Payment is continuous until I email to change or terminate.
• $3.00 fee per card charge & decline. No ACH fee, but $20 decline charge.

DCYF Working Connections & Best Starts for Kids

• I will provide verification of current subsidy before joining & promptly reauthorize annually and/or per DCYF/BSK w/in the month.
• I will contact Vashon Kids Admin to communicate technical issues and support needs before my subsidy expiration.
• If I fail to renew my authorization before DCYF terminates my account, I will accrue and be personally responsible for payment of my child's tuition balance to VYFS. We are happy to provide scholarship forms & payment plans by request

IN SIGNING I agree to adhere to payment terms, costs, & conditions, with proactive communication of payment or subsidy difficulty. I understand summer cancellations require 3-WEEK notice to waive tuition responsibility., and that lack of compensation and/or written plan may lead to loss of my child's spot in care.

Payment Agreement Signature *   

Participation: I authorize my child's participation in all VYFS activities, including but not limited to on-campus forest play & theater club in the CES music room. VYFS may collect & use any media of my child for internal, and/or promotional purposes, and in the event of celebrations & parties, my child may wear nontoxic temporary tattoos, facepaint, and/or stickers. 
School Exchange of Information: With all information confidential per FERPA, I authorize communication by VYFS/Vashon Kids & VISD #402 on behalf of my child's education & growth through the year. I may withdraw consent in writing at any time (excl. info previously exchanged)
Medical treatment: I authorize CPR & 1st aid by qualified staff. If I cannot be contacted, I permit my child's transport by aid vehicle to emergency services, disclosure of health information & medical, surgical, and/or hospital treatment & procedures (eg. necessary tests, x-ray, transfusions, injections, drugs), accompanied by an employee & performed as deemed immediately advisable by a licensed medical provider.
Release from Liability: While VYFS does its best to ensure a safe experience, I understand & agree to assume the risks of accidents that may occur from participation in or transportation between program activities. I release VYFS, its employees, volunteers, contractors, directors, & agents from all liability based on any damage, loss, or injury caused to my child or me by participation in VYFS programs.
I understand and agree:
*I am the business partner & data recipient eg financial, attendance, enrollment, regarding my child's care.
*Any of their legal guardians may join a child in the program, w/possible screening. If detrimental to care/safety, may be limited.
*Guardians & authorized pickups must always log the time & sign each dropoff/pickup. Children leave only w/ preauthorized persons, and must never be left at VYFS or Vashon Kids sites without childcare staff. For safety, if a person arrives to pick up my child & appears intoxicated, staff may have no recourse but to contact police.
*Childcare staff are mandated by law to report suspected abuse/neglect for investigation & are protected from liability for good faith reporting. Non-mandated staff are also encouraged to report.
