I hereby offer my services as a VOLUNTEER on the vessel BILL OF RIGHTS, owned by the South Bayfront Sailing Association. I will receive no compensation for my services. I may accept or not, a reimbursement for incidental living expenses in an insignificant amount.
TERMS AND CONDITIONS
This Agreement is valid when signed and returned and acceptance of by South Bayfront Sailing Association. Deckhand status is contingent upon a signed crew medical form attesting to your medical suitability for the crew duties indicated, results of the USCG required drug screening and other requirement as determined by the organization and USCG.
I, the undersigned, understand represent that I am in good physical condition, free of signs of illness, free from any physical constraints and/or injuries and not under any medications that could prevent me from performing the duties as a crewmember. I understand that smoking and alcohol aboard is restricted, and may be forbidden during certain programs. I understand that upon signing this agreement I am bound and obligated to the terms as stated herein and the Code of Conduct and other guidelines laid out by the Master of the Vessel and available in the ships SMS Manual and Crew Handbook. I understand my failure to abide by the terms herein may result in fines and/or penalties as provided by law (46 U.S.C §11501, et.seq.).
