  • Application Instructions:   Please complete the following form if you would like to enroll students you will advise in the HART Initiative for the 26-27 school year. The deadline to complete this form is Friday, August 28, 2026.

  • HART Ambassadors are required to complete six individual projects or one long-term project and progress reports detailing those projects throughout the school year to earn recognition. Will you encourage your students to complete these requirements?*
  • School Information Please complete the following section and provide essential information about your school. Schools must participate in National School Lunch Program in order for students to be eligible to be a Health Ambassador.
