REFUND POLICY: Registrations are NON-REFUNDABLE within 7 days of the program start date and once the program has started. Refunds requested 14+ days before the start date are subject to a $25 admin fee. Refunds requested 7 to 13 days before the start date receive 50% refund or full program credit. If KD Elite Football cancels the full program, I am entitled to a full refund or program transfer option.
LIABILITY WAIVER: I release KD Elite Football, its coaches, staff, and facility owners from any and all liability for injuries, damages, or claims arising from my player's participation in camp activities. I assume all risks associated with participation.
PHOTO CONSENT: I grant KD Elite Football permission to use photos and video of my player taken during camp for promotional purposes on social media and marketing materials.
MEDICAL AUTHORIZATION: I confirm my player is in good health and physically fit to participate in soccer training activities. I have disclosed all known allergies, medical conditions, medications, and injuries on this registration form. I authorize KD Elite Football coaches to provide basic first aid if needed and to contact emergency services if my player becomes injured or ill during camp. I am responsible for providing any required medications or medical equipment.
