본인(부모/대리인)은, 집회(라이드 포함)의 모든 활동이 적절한 감독과 보호에 따라 이루어지고 있음을 이해하며, 위 학생이 집회에 참여하는 동안 발생할수 있는 사고 및 상해에 대하여 KOSTA/USA와 Northbay Adventure Camp 측에 어떠한 법적 책임을 요구하지 않기로 동의합니다. 사고 및 상해로 인한 손실에 대하여 다른 보험이 확정되지 않은 때에, 본인은 위 학생에 대한 법적인 책임을 지기로 동의합니다.
본인은, 사고나 응급 치료시 KOSTA/USA 측에서 필요한 절차를 행하는 것을 동의합니다. 필요에 따라 위 학생에게 약을 주는 것을 허락합니다.
본인은, 위 학생이 집회의 안전과 진행에 반하는 행동을 했을 때에 집회 참여 부분 제한, 혹은 심각한 경우 집회 참석 중지를 요청받을 수 있음을 이해하며, 이와 관련된 집회 측 연락에 상시 대기할 뿐 아니라 집회 측의 요청시 학생을 집회 장소에서 즉각적으로 인계하기로 동의합니다.
I understand that this is a voluntary service that is provided by KOSTA/USA, but the continued care and well-being of my youth is still my sole responsibility. Therefore, in consideration of your providing this service, I hereby release, discharge and hold and save harmless KOSTA/USA, their respective officers, directors, employees and agents, and each of them from and against any and all liability, claims and damages resulting in any way from my youth participating in Youth KOSTA Conference. I understand that it is still my responsibility for the safety and health of my youth even though he/she may be a participant in the Youth KOSTA Conference.
I hereby authorize KOSTA/USA, in the event of misconduct, emergency, accident or illness, to take whatever steps they deem necessary or appropriate, including the obtaining of medical or emergency treatment for my youth. In the event of misconduct, I understand my youth may be removed and not allowed to further participate in the program. Any costs incurred as a result of misconduct, emergency or illness shall be paid or reimbursed by me.
Furthermore, KOSTA/USA is authorized to allow my youth to come or go from the program as requested by me. If my youth is to be picked up and no one arrives, KOSTA/USA is authorized to take whatever action deemed necessary and appropriate in consultation with my youth.
