    Medicare needs a GP medical doctor's referral.

    A dentist can recommend the study but can't refer you.

    Two Choices

    • Opt in to have us obtain this GP medical referral for you and our GP does it now. You will see the cost then if you are not having your own GP give you the referral because you have been referred by your own GP medical doctor, and not a dentist.

    If your dentist recommended this study for teeth grinding, choose this option.

    You can also add it if you just want it. You pay this when you make your reservation, not today.

    Without a Medicare card — you pay for the study: $441.10, or $510.10 with teeth grinding included.

     

  • If you have an Australian Medicare card

    Please write your LEGAL name as it appears on your medicare card exactly.

    If you do not have a Medicare card, as your legal name appears on your other Australian legal identification documentation.

    It must match your Medicare record if you expect to be bulk billed. We ask for it on both the initial details form and medical history form, and check that they agree.

    If they don't match, your sleep test will be delayed while we sort it out.

    Reciprocal cardholders are NOT eligible for a Medicare claim for this sleep study — you would need to pay privately if you proceed with a booking.

  • Medicare will only cover your sleep study if a medical doctor refers you. You can use your own GP, or we can arrange it for you through Dr Manish Shah, and you pay the $20 here on this form.

    Dr Shah has a commercial relationship with Aer Healthcare. The $20 referral fee is the only thing you pay here. Anything else you choose is paid when you book.

    Why do we need your GP's details?

    We will add your regular GP's details to the referral form that Dr Shah signs, if you choose to pay for Dr Shah's referral.

    We still need your manual input for the name and other details of your GP so that we can double check the  accuracy of the details.

    • Teeth grinding (bruxism)

      Bruxism means grinding or clenching your teeth while you sleep. Most people don't know they do it — usually it's a dentist or doctor who notices the wear on the teeth and mentions it.

      If your dentist or doctor has talked to you about grinding, clenching, worn or cracked teeth, jaw or face pain, or a night guard or splint, this is the analysis they're likely wanting.

      If nobody has raised it with you, you probably don't need it.

      You are not paying anything now. Nothing is charged until you book. So if the medical history shows Medicare won't cover you and you'd rather not go ahead as a self-funded study, you can stop there and there is nothing to refund.
