I understand and agree that:
- My child will identify themselves in the video using first name and age only.
- The video may be reviewed by contest judges.
- Submitted videos may be posted or shared on Chicagoland Food Science Foundation websites, social media, or educational materials to promote the contest and food science education.
- No last names, addresses, school names, or other personal identifying information should be included in the video.
Eligibility Confirmation

- My child is 12–18 years old.
- My child lives within the Chicago Section IFT geographic region.
- The video submitted is original work created by my child.
Media Release

I grant permission to the Chicagoland Food Science Foundation (CFSF) to use my child’s video submission, image, or voice for educational, promotional, and non-commercial purposes related to the contest and Chicagoland Food Science Foundation Marketing.

Prize Acknowledgment

I understand that eight (8) winners will be selected to attend a free Pizza & Culinary Academy hosted by the Chicagoland Food Science Foundation. Additional information about Pizza & Culinary Academy logistics will be provided to selected winners.
