    Godparents must be Catholic (If only one Godparent; Godfather for boy, Godmother for girl)

  • NO BAPTISM WILL BE SCHEDULED UNTIL ALL FORMS AND CERTIFICATES HAVE BEEN RECEIVED AT OUR PARISH OFFICE.

  • Birth Certificate of child/adult being baptized must be emailed to ccd@stmichaels77.org at time of registration. 

    Godparents need to each sign the Sponsor/Godparent form, see separate attachment, and emailed to ccd@stmichaels77.org

    Parents and Godparents must attend Baptism Class.  For Baptism Classes in-person, reservations need to be made one week prior to class or there will be no class. Please call the office 520-723-6570 or email ccd@stmichaels.org to make your reservations. 

  • If you have more than one child/adult to be Baptized, please submit a registration for each child/adult to be Baptized.

    All certificates and forms must be submitted and received at our Parish Office before scheduling a Baptism date.
