1. An Undergraduate Sponsoring Chapter, Professional Sponsoring Chapter, or a committed Advisor

2. Chapter Information (School, Region, Mailing Address, Chapter Email, Telephone, Number of Members)

3. Chapter Leadership Information (Executive Board email address', President address, telephone and email, and Advisor name, title, address, email and telephone)

4. Chapter Representative Contact Information (Name, address, email and telephone of representative from sponsoring chapter or Advisor if chapter is NOT sponsored)
To complete this New Student Chapter Application Form you will need:

1. Chapter Information (School, Region, Mailing Address, Chapter Email, Telephone, Number of Members)

2. Chapter Leadership Information (Executive Board email address, President address, telephone and email, and Advisor name, title, address, email and telephone)
