The purpose of this form is to give us as much information as possible about your party, your guests, and of course the one and only birthday star! It will give us everything we need in order to customize your party to your needs and interests!

These questions will be used to formulate the special, imaginary birthday adventure we will have on Zoom! They will help our birthday specialists determine where we will go on our adventure, who we will meet there, and what we will do at your party! Please note: We cannot provide parties that include licensed characters from movies or TV shows. However, we can visit some of their friends and tie in those themes!

Where would the birthday star like to go on the "imaginary adventure" part of their party?
