Wyoming Dachshund and Corgi Rescue is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. Wyoming Dachshund and Corgi Rescue is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care, which includes worming, vaccinations, spaying/neutering (for dogs 6 months or older) and microchipping prior to their adoption.

This questionnaire is designed to help Wyoming Dachshund and Corgi Rescue and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Wyoming Dachshund and Corgi Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

I give Wyoming Dachshund and Corgi Rescue, Inc. permission to contact my veterinarian, references, make follow-up calls, and make a home visit. If, for any reason, I cannot keep this dog, I will contact Wyoming Dachshund and Corgi Rescue, Inc. and arrangements will be made for the dog to be returned.

Therefore, I hereby release Wyoming Dachshund and Corgi Rescue, Inc., its heirs and assigns, from liability for any injury or damages to any person or property caused in the future by said rescue and from any causes of actions, claims, suits or demands whatsoever that may arise as a result of such injury or damages.
