    Verification Worksheet for Dependent Students

  • Your Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification.  The law says that, before awarding federal student aid, we may ask you to confirm the information you and your parent reported on your FAFSA.  To verify that you provided correct information, Student Financial Services will compare your FAFSA information with the information on this worksheet and any other required documents.  If there are differences, your FAFSA information may need to be corrected.  You and one parent must complete, print and sign this worksheet and submit it and other required documents to our office.  We may ask you for additional information upon receipt of this form.

  • In the spaces below, list the people in your parent(s)' household as described in each section.

  • Yourself and your parent(s).  If your parents are divorced, list the custodial parent (and stepparent if applicable) as listed on the FAFSA.  List college for a parent only if s/he will be enrolled at least half-time during the {aidYear} school year.

  • Your parent(s)' other children if your parent(s) will provide more than half of their support from July 1, {StartingYear}, through June 30, {EndingYear}, or if the other children would be required to provide parental information if they were completing a FAFSA for {aidYear}.  Include children who meet either of these standards even if the children do not live with the parent(s).  List college for a child only if s/he will be enrolled at least half-time during the {aidYear} school year.

  • Other people if they now live with your parent(s) and the parent(s) provide more than half of their support and will continue to provide more than half of their support through June 30, {EndingYear}.  List college for an individual only if s/he will be enrolled at least half-time during the {aidYear} school year.

  • C.  Dependent Student's Income Information to be Verified

  • You will need to submit a {TaxYear} IRS Tax Return Transcript, which may be ordered at www.IRS.gov or 800-908-9946, in order to provide documentation of your tax return information.

  • In order to provide documentation of your tax return information, you will need to submit the following:

    • A {TaxYear} IRS Tax Return Transcript, that will only include information from the original tax return and does not have to be signed, which may be ordered at www.IRS.gov or 800-908-9946; and
    • A signed copy of the {TaxYear} IRS Form 1040X "Amended U.S. Individual Income Tax Return" that was filed with the IRS.
  • An individual who was the victim of IRS tax-related identity theft must provide a Tax Return Database View (TRDBV) transcript obtained from the IRS, or any other IRS tax transcript(s) that include all of the income and tax information required to be verified.  Please contact the IRS Identity Protection Specialized Unit (IPSU) at (800) 908-4490.

  • In order to provide documentation of your tax return information, you will need to submit a transcipt that was obtained at no cost from the relevant taxing authority of a U.S. territory (Guam, American Samoa, the U.S. Virgin Islands) or commonwealth (Puerto Rico, the Northern Mariana Islands) or a foreign central government that includes all of the tax filer’s income and tax information required to be verified for tax year {TaxYear}.  This transcript must contain the following {TaxYear} information, as applicable:

    • Income earned from work (wages, business, farm)
    • Federal income taxes paid
    • Adjusted Gross Income - This is the total income plus interest and dividend income minus housing allowances and minus contributions to retirement accounts.  (Housing allowances and contributions to retirement accounts are reported separately on the FAFSA.)
    • Untaxed portions of IRA or pension distributions
    • Deductions for IRA contributions
    • Tax-exempt interest income
    • Non-refundable educational tax credits

    If a free transcript is not available, you may instead submit a signed copy of your foreign tax return which was submitted.

    After receiving the tax transcript, please label and translate the relevant tax lines which most closely correspond to the tax items listed above on an IRS tax return.  Please also write next to each figure the amount converted to U.S. dollars based upon the exchange rate the day the FAFSA was originally submitted.

  • Please list below the names of all the student's employers, the amount earned from each employer in {TaxYear}, and whether an IRS W-2 form was provided by the employer (attach copies of all {TaxYear} IRS W-2 forms issued to the student).  List every employer, even if the employer did not issue an IRS W-2 form.

  • D.  Parent(s)' Income Information to be Verified

  • E.  Certifications and Signatures

  • For timely disbursement of your financial aid, verification documentation must be received by June 1, {StartingYear}.  If your financial aid disburses after your semester payment is due, you will be subject to late fees and interest charges.  However, you must provide verification documentation prior to the last day of your enrollment for campus-based, Direct Loan and institutional need-based aid.  Verification documentation must be provided for Federal Pell Grant applicants prior to 120 days after the last day of your enrollment.

  • Each person signing below certifies that all of the information reported is complete and correct.  The student and one parent whose information was reported on the FAFSA must sign and date.
