TREATMENT OVERVIEW
Dermal fillers are injectable treatments used to restore volume, smooth wrinkles, enhance facial features, and stimulate collagen production. Aeris Medical Aesthetics uses only FDA-approved products including hyaluronic acid-based fillers (Juvederm®, Restylane®, Revanesse®), SkinVive™ for skin hydration, and Sculptra® for collagen biostimulation.

⚠️ PRE-TREATMENT INSTRUCTIONS
To minimize risk and improve results, follow these guidelines 3–7 days prior to your appointment:

❌ Avoid NSAIDs (ibuprofen, aspirin), Vitamin E, fish oil, and alcohol for 48–72 hours
❌ Discontinue Retin-A or glycolic acid 2–3 days prior
❌ No dental procedures, vaccinations, or major medical treatments for 2 weeks before or after
✅ Inform us if you have a history of cold sores—an antiviral may be prescribed
✅ Stay well hydrated
✅ Eat a light meal before your appointment
❌ Do not schedule around events where swelling or bruising would be problematic

🩹 POST-TREATMENT INSTRUCTIONS
It’s normal to experience mild swelling, bruising, tenderness, or redness. These may last 1–14 days depending on product and area treated.

✅ Do:

Apply cold compresses for 10–15 min/hour for the first 6 hours
Keep head elevated and sleep on your back the first night
Use gentle skincare and SPF
Take arnica or bromelain (optional) to reduce bruising
❌ Avoid:

Alcohol, intense exercise, or hot environments (saunas, tanning) for 24–48 hours
Massaging or pressing on treated areas unless instructed
Dental work or vaccinations for 2 weeks
Makeup application for 12 hours post-treatment

Hyaluronic Acid Fillers (Juvederm®, Restylane®, Revanesse®):

Immediate volume results
Best results within 1–2 weeks
May last 6–18 months depending on area and product
SkinVive™ (Juvederm):

Targets skin quality and hydration
Effects improve over 1–4 weeks
Minimal downtime
Sculptra®:

Stimulates natural collagen over time
Results are gradual (4–12 weeks) and long-lasting (up to 2 years)
Requires a series of treatments (typically 2–3 sessions)

⚖️ RISKS AND SIDE EFFECTS
I understand that dermal fillers are medical procedures and carry potential risks, including:

Swelling, redness, bruising, tenderness
Lumps, nodules, or asymmetry
Infection, scarring, delayed hypersensitivity
Vascular occlusion or tissue necrosis (rare but serious)
Unsatisfactory results or need for touch-up treatments

I understand that although positive outcomes are expected, no guarantees—express or implied—can be made regarding the success, duration, or satisfaction of any treatment or service I receive at Aeris Medical Aesthetics. I acknowledge that all services offered are elective in nature, are not intended to diagnose, treat, cure, or prevent any medical condition, and that results will vary from person to person.

I understand and accept that:

All sales are final. Aeris does not offer refunds on any service, procedure, treatment packages, or retail product.
Optimal outcomes may require a series of treatments and/or ongoing maintenance, based on individual response.
Treatment results may be affected by my personal medical history, medications, lifestyle, metabolism, skin condition, or other biological factors.
No promises, warranties, or guarantees have been made regarding outcomes or satisfaction.
I voluntarily accept and acknowledge the potential risks and side effects associated with aesthetic and wellness treatments, which may include, but are not limited to:

Redness, swelling, bruising, discomfort, or tenderness
Scarring, hyperpigmentation or hypopigmentation
Infection, allergic reaction, or inflammatory response
Temporary or prolonged skin sensitivity
Unsatisfactory results or need for additional treatment(s)
I agree to disclose all relevant medical history, medications, allergies, and previous procedures to ensure safe treatment planning. I also agree to follow all pre- and post-care instructions provided by Aeris Medical Aesthetics.

By signing this waiver, I hereby release and hold harmless Aeris Medical Aesthetics, its owners, medical directors, providers, employees, agents, and affiliated personnel from any and all liability for injury, adverse reaction, dissatisfaction, loss, damage, or claim arising from or related to any service rendered, except where prohibited by law.

I understand that Aeris Medical Aesthetics maintains a professional, respectful, and safe clinical environment. Disruptive, aggressive, or inappropriate behavior may result in immediate dismissal and denial of future services. Full payment will still be required.

This agreement will remain in effect for all current and future treatments received at Aeris, unless revoked in writing by me.
