This form must be submitted prior to the date of the trip in order to confirm your vehicle registration. Public Safety staff will email you to confirm the requested vehicles availability and reservation.
YOUR RESERVATION IS NOT CONFORMED UNTIL YOU RECEIVE YOUR CONFIRMATION EMAIL
Vehicles may be picked up and returned at the Department of Public Safety.
Please Note:
1. All drivers of College owned, leased and rented vehicles must be certified through the Department of Public Safety to operate said vehicles.
2. College vehicles are not for personal use or any purpose other than official college business. Insurance may not cover vehicle damage or personal injury for other than authorized use.
3. The driver(s) agree to obey all policies, laws and ordinances goverining the use and parking of motor vehicles. Driver(s) are personally responsible for any violations, fines or charges incurred for violations.
4. If gasoline credit cards are issued, the driver(s) are personally and financially responsible for any unauthorized use.
5. For trips outside the Richmond/Wayne County area, a list of passengers (to include emergency contact numers) is required before departure; and, a cell phone must be available and listed on the passenger list for emergency communications.
6. Any accident involving a College owned, leased or rented vehicle should immediately be reported to the Department of Public Safety at 765-983-1400.
