🔹 PRE-TREATMENT INSTRUCTIONS (ALL BBL TREATMENTS)
Please follow these instructions to ensure your safety and best possible results:

✅ 2 Weeks Before Treatment
Avoid sun exposure, tanning beds, and self-tanners
Discontinue:

Retinols, AHAs/BHAs, Vitamin C serums
Hydroquinone, benzoyl peroxide, and exfoliants
No waxing, threading, tweezing, or depilatory creams on treated area
Let your provider know if you've had recent Botox, filler, or chemical peels
✅ 48–72 Hours Before
Discontinue aspirin, ibuprofen, alcohol, and fish oil (if medically safe)
Stop using any topical irritants on the area
Shave the area the night before your appointment (for LHR only)
✅ Day of Appointment
Arrive with clean, dry skin (no lotion, makeup, SPF, deodorant, etc.)
Wear loose, comfortable clothing
Bring a list of all medications and supplements

🔹 POST-TREATMENT INSTRUCTIONS
Your skin may feel warm, look red or flushed, or show temporary swelling after treatment. These effects are normal and typically resolve within 24–72 hours.

✅ Care After Any BBL Treatment
Apply cool compresses, cold aloe vera, or thermal water spray to soothe skin
Use gentle cleanser and moisturizer — avoid scrubbing
Apply SPF 30+ daily and reapply if outdoors
Avoid heat, sweating, saunas, and hot showers for 48 hours
Do not pick, exfoliate, or peel dark spots or flaking areas — they will shed naturally
Avoid sun exposure until skin is fully healed
⚠️ What to Expect

📝 CONSENT FOR SCITON BBL TREATMENTS (Multi-Use)
I authorize Aeris Medical Aesthetics and its designated providers to perform BroadBand Light (BBL) treatments using the Sciton platform. I understand that BBL is a light-based technology intended to treat a variety of skin conditions, including pigmentation, vascular lesions, acne, signs of aging, and unwanted hair.

By signing below, I acknowledge and accept the following:
Nature of the Procedure
BBL emits pulses of light absorbed by pigment, blood vessels, acne bacteria, or hair follicles depending on the treatment protocol selected.
Treatment Goals
I understand that multiple treatments may be required and that results vary by skin type, condition, and compliance with pre- and post-care.
Risks and Side Effects
Possible side effects include:

Redness, swelling, warmth (common)
Blistering, scabbing, or temporary bruising
Pigment changes (hyper/hypopigmentation)
Burns or scarring (rare)
Reactivation of cold sores (if prone)
Medications & Contraindications
I have disclosed all medications, including antibiotics, Accutane, and any photosensitizing drugs or skincare.
Pregnancy/Breastfeeding
I confirm I am not pregnant or breastfeeding, or have discussed treatment risks with my provider.
Provider Disclaimer
Treatment will be performed by a trained medical professional. Aeris reserves the right to defer or deny treatment if medically necessary.
No Guarantee of Results
While BBL has been shown to be safe and effective, results are not guaranteed. I understand that photos may be used for documentation and education.

AERIS MEDICAL AESTHETICS – CLIENT CONSENT & LIABILITY WAIVER
I understand that although good results are expected from the services provided at Aeris Medical Aesthetics, there can be no guarantee, warranty, or assurance—express or implied—that I will be completely satisfied with the outcome, or that I will not require additional or ongoing treatments to achieve or maintain the desired result.

I understand that:

These treatments do not cure medical conditions nor provide immunity against the recurrence of such conditions.
Treatment results are temporary and vary by individual, with some clients experiencing shorter or longer-lasting effects.
The number of treatments required depends on multiple factors including, but not limited to:

Severity of the condition being treated
Degree of volume loss, pigmentation, or skin irregularity
Patient age and medical history
Metabolic rate and healing response
History of prior treatments, trauma, or surgery
Lifestyle factors such as sun exposure, smoking, or skincare compliance

Financial and Policy Acknowledgments
I understand that all product and service sales are final.
Aeris Medical Aesthetics does not offer refunds on services or retail purchases.
I have been given the opportunity to ask questions and have them answered to my satisfaction before signing this consent.
I fully understand the procedure and the associated risks, and I voluntarily accept those risks.

Assumption of Risk and Release of Liability
By signing this document, I acknowledge and agree to the following:

I consent to receive services including, but not limited to: injectables, laser treatments (including BBL), facials, microneedling, medical-grade skincare, waxing, or other cosmetic or medical services offered by Aeris Medical Aesthetics.
I understand that these procedures carry certain risks, and I voluntarily assume the risk of injury, adverse effects, dissatisfaction, or complications that may result from any treatment performed.
I certify that I have fully disclosed all pertinent medical information, including medical devices or implants, medications, allergies, or conditions that may impact my treatment outcome.
I understand that failure to disclose medical conditions, devices, or medications may increase my risk of unwanted or unexpected results.

Limitation of Liability
Except where prohibited by law, I agree that neither Aeris Medical Aesthetics, its staff, owners, affiliates, providers, medical director, nor anyone acting on behalf of the practice shall be liable for any injuries, adverse effects, economic loss, emotional distress, or property damage that may arise from my treatment or use of the facility.

I hereby release Aeris Medical Aesthetics, its employees, contractors, medical staff, and any authorized agents from all claims or liabilities of any kind, including personal injury or property damage, sustained during or as a result of services received at or through the facility.

This waiver applies to me, my heirs, executors, representatives, or assignees.

Professional Conduct and Expectations
Aeris Medical Aesthetics maintains a professional, safe, and respectful environment.
Any inappropriate, aggressive, or harassing behavior may result in immediate termination of services and full payment for scheduled treatments will be required.

Consent Duration and Validity
This agreement remains valid for all current and future services performed at Aeris Medical Aesthetics unless revoked or amended in writing by either party.
I understand that I may request a new consent form at any time.
