Special Day Limo Service Agreement

Welcome to Special Day Limousine, LLC. Thank you for choosing us to help you make your special day a great success! Please fill out your information, read terms carefully and sign. You can then submit deposit using your credit card.
Your limousine has been inspected by your driver and/or by our management prior to our engagement. Prior damage has been noted and recorded for your protection and safety. You (the customer) are encouraged to inspect and confirm the condition of your limousine prior to use. If you notice any damage to the interior or exterior of the vehicle, you should note that on the Damage Form that your driver will provide you prior to departing. This car will be re-inspected upon conclusion of our engagement, by you and our driver. Any new damages will be your responsibility. If there are any missing or broken glasses there will be $15.00 applied to your credit card per glass. Your credit card will be charged for all repairs and any unusual cleaning that was caused by you or anyone in your party.
A minimum deposit of $150.00 is required to secure the date of your engagement. All deposits will be put toward the final bill. All bookings must have a credit card on file with sufficient credit from which we will draw your reservation deposit, rental and overtime fees, and any applicable damage or cleaning charges automatically. All deposits are NON-REFUNDABLE regardless of cancellation. If there is a weather issue, we will gladly work out rescheduling your booking.
IMPORTANT!! You will pay a $150.00 charge for each passenger that vomits in or on the limousine or tries to smoke in the limousine. There will be a separate charge for each incident. There will be a $150.00 cleaning charge if any cupholders are used for spitter cups. The driver will be more than happy to pull over or stop at any given time when asked to do so, as long as the driver determines it is safe to do so in his sole discretion.
