    YOUR REQUEST WILL BE PROCESSED BASED ON YOUR AGREEMENT TERMS.

    While we hate to see you leave the family, we understand that canceling your membership may be the right decision for you at this time. To cancel your membership, please read the instructions below.

    After completing this form, please allow 7-10 business days to confirm your request.  Business days are Monday – Friday excluding holidays.  Some requests may require additional information.

    Your submission date (provided all necessary information is included*) is the date used to process your request. Requests are not finalized until all required information is received. Incomplete request(s) will be voided. The effective date of cancellation is the date your membership is terminated.

    NOTE: If you are uncomfortable coming to the gym due to COVID-19, you have the option to FREEZE your membership for the months of July and August.

      If during the term of your membership you become physically unable to use a substantial portion of the health club services for 30 or more consecutive days, you may cancel your membership with a doctor’s or nurse practitioner’s certificate.

      Have your doctor or nurse practitioner complete the Verification of Disability Form.

      • Medical Disability or Death - If primary member is unable to use a substantial portion of the membership for 30 or more consecutive dates, with a completed  Verification of Disability Form (as found at Verification of Disability form) the membership will be canceled upon receipt. In the event of buyer’s or primary member’s death, the buyer/buyer’s estate shall be relieved of any further payments upon receipt of death certificate.
      • Your effective date of cancellation for medical cancellations is the date American Family Fitness receives complete documentation (including the $100.00 relocation fee, if applicable) per cancellation polices found on your membership contract.
      • In the event your membership dues or additional services were drafted after American Family Fitness receipt, a refund will be presented to your financial institution less the amount equal to the exact value of services rendered.


      Cancellation received on the date of membership billing will not be considered for refunds. Your effective date of cancellation will be 30-days after your last membership dues billing.
      In rare circumstances in which payment has not been received for services rendered, a balance will be placed on your account and an invoice will be mailed the address on file.

    • RELOCATION

      If during the term of your membership you permanently relocate 25 miles away from any American Family Fitness, your membership will be terminated with valid proof of relocation, $100 cancellation fee and a Request to Cancel Form.

      Valid proof of relocation includes: Primary member’s name, a physical address (PO Box not accepted) and the current date (within past two months)

      • RENTAL OR LEASE AGREEMENTS ARE NOT ACCEPTABLE.
      • Drivers license, not more than two months old
      • Deed, mortgage, monthly mortgage statement, not more
        than two months old
      • Utility bill, not more than two months old, issued to the primary member. The service and mailing address must be the same. Examples include gas, electric, sewer, water, cable or phone bill.
        (Cellular phone and pager bills are not accepted.)
      • Copy of current voter's registration receipt at primary member’s new district
      • Payroll check stub issued by an employer within the last two months with both primary member’s name and address imprinted
        Current homeowners insurance policy, automobile or life insurance bill (Cards or policies are not accepted.)
      • Canceled check not more than two months old with both name and address imprinted
      • Certified copy of school tuition with room/board issued by a school in which the member is currently enrolled and is accredited by a U.S. state, jurisdiction or territory (A report card is not accepted.)
      • An affidavit from attorney
        • Affidavit must be on attorney’s letterhead
        • Affidavit can only be signed by the attorney
        • Make sure that it includes the 3 required items: member’s name, member’s new address and date member moved.

    • MILITARY

      American Family Fitness recognizes the sacrifice of our military members. Military deployment orders or permanent change-of-station that affects the member’s ability to utilize the club.

      If you are being deployed, or if you are being transferred, with a copy of your orders containing a reporting date, your membership will be canceled in respect to your reporting date and membership bill date. If your membership obligation was pre-paid, membership will be canceled on the reporting date and refunded to the account on file or a check mailed to the address above. We do not charge the $100 relocation fee for our military members reporting for duty.

      Any scheduled payments due prior to your effective date of cancellation will be presented to your financial institution for payment; including, but not limited to your yearly maintenance fee. Your effective date of cancellation is 30-days after your last membership payment. 

      The effective date of cancellation is the date your membership is terminated. Any payments and services due before your effective date of cancellation will be presented to your financial institution for payment. Member is responsible for any billing that will occur within the next 30 days. No refunds.
