  • Please complete by February 9, 2021. Once you hit submit, it will be sent. 

    If you have a very large session and would prefer to email a word or excel document with all of the elder's information, that is also acceptable.  You can email documents to Adrien Domske (412-697-9207) at adomske@pghpresbytery.org, or contact her if you have any questions about the form.

    IMPORTANT NOTES:

    • Please use the TAB button to move between entries.  If you hit the ENTER button, it will accidentily submit the form before you have completed your work.
    • If you want a copy of your form for your own records, please hit the PRINT button BEFORE you submit. From your print screen, most systems should also have the option to save to a PDF for your files. Once you have saved and/or printed, THEN hit submit. Once you submit, you won't have access to the form again.
    • Once you start the online form, your input will automatically be saved locally on the browser (Firefox, Internet Explorer, Google Chrome, etc.) storage for up to 24 hours. The saved local data is cleared when the form is submitted or 24 hours after the form was started. This way, you will not lose any data if you accidentally close or reload your browser while filling out the form. Please note that you need to use the same browser in order for the saved field values to autofill in the form.
