2nd Grade Field Trip Permission Slip: Woodruff Art Center

Please fill this form out before your scholar attends the trip.
A field trip is scheduled for our Kindergartners to vist Woodruff Art Center located at 1280 Peachtree Street NE, Atlanta, GA 30309. Please keep the follwoing in mind:
- Trip Date: Tuesday, April 14th
- Trip Cost: $20.00 (per scholar)
- The group will depart at 9:00 AM and is estimated to return at 1:00 PM.
*DIA will not accept verbal approval*

**This form must be completed by the parent/guardian. Payment can be made in Infinite Campus**

I understand that my child will be traveling to and from by bus. I also authorize, in case of an emergency, necessary medical treatment(s).
