ACKNOWLEDGEMENT, WAIVER AND GENERAL RELEASE
My child will be participating in FAMcamp, sponsored by the First Americans Museum Foundation (“FAMF”), operator of First Americans Museum (“FAM”).
I understand that participating in a workshop may carry inherent risks. I understand that factors beyond anyone’s control, including negligence, may affect my child’s safety and well-being. In signing this Acknowledgement, Waiver, and General Release, I acknowledge that I allow my child to participate willingly and assume all risks associated with my child’s participation.
If injured, my family will rely solely on our own insurance or resources to cover any medical bills or other expenses or losses. I understand that no workers’ compensation or third-party insurance will be available to my family from FAMF.
On behalf of my family, I hereby release and promise not to sue FAMF and its officers, directors, agents, representatives, employees, contractors, affiliates, successors, and assigns (collectively, “FAMF Group”) from any and all liability for any claims or losses that I now have, ever had, or may ever have, related, in any way, to my child’s activities in relation to FAMcamp. Further, I agree to indemnify and hold harmless each member of the FAMF Group from any and all liabilities or claims made as a result of or in any way related to my child’s participation in FAMcamp, whether caused by negligence or otherwise. I intend that this Acknowledgement, Waiver, and General Release be construed broadly to the maximum extent permissible under applicable law.
In the event of a medical emergency, I hereby give FAMF my permission to secure treatment for my child and, if necessary, to secure my child’s transportation to the nearest suitable hospital, all at my expense.
