Programming for individuals under 18 years of age is separated into 3 options:
Child Care - Ages 0-4 (on-site) - Infants and toddlers will be cared for by professional childcare providers with the assistance of capable community volunteers. Kids Camp - Ages 5-12 (off-site; transportation provided) - School-age children will travel by bus to Splash Universe in Dundee, MI for a fun-filled day at the waterpark, returning in time to witness Animal Magic in the Auditorium.
IMPORTANT: Space is limited! Kids programming for ages 5-12 (Splash Universe water park) will be on a first-come, first serve basis. HFM reserves the right to coordinate alternative children's programming for overflow registrations.Teen Program - Ages 13-17 (off-site; transportation provided) - Teens will meet in conference room D for a workshop with a national speaker before traveling by bus to Zap Zone for a round of laser tag and returning in time for Animal Magic in the Auditorium.
There will be programming for each age group on Saturday during the day.

SpringFest health staff CANNOT administer medication during the program except for first aid/emergency purposes. Please plan your child's medication schedule accordingly.
If your child has a chronic health issue that is not related to a bleeding disorder, a parent or guardian or an appointed parental representative (age 21+) may accompany your child to care for the special need.

Section A:By signing below, I give HFM (Hemophilia Foundation of Michigan) and/or its assignees all rights, title, and interest in whatever photographs and/or video footage in which I/my minor child may appear, for the purposes outlined below, without compensation to me.
I further authorize the use of my/my minor child’s statements and testimonials, narration of services provided to me, narration of service outcomes, articles, stories, or other such literary, artistic, or educational value, and consent to the copyright and fair use of same, in accordance with all applicable trademark and copyright laws.
I further release HFM, its Board, its officers and representatives from any and all claims of any nature arising from any medium and/or publication.
Section B:This authorization and assignment is intended for, but not limited to, the publication of promotional documents, pamphlets, brochures, news articles, charitable drives, website use, social media, agency advertising, television, and other such marketing materials.
I therefore give the HFM (Hemophilia Foundation of Michigan) the absolute right and permission with respect to items listed in Section A to use, re-use, publish and re-publish the same in whole or in part, separately or in conjunction with other information, photographs, video, etc. in any medium now or hereafter known, and for any purpose whatsoever, including but not limited to illustration, promotion or advertising.
Section C:I hereby affirm that I am of full age and have the right to contract in my own name or on behalf of my minor child. I have read the foregoing and fully understand the contents hereof. This release shall not expire and will be binding upon me and my heirs, legal representatives, and assignees.

PLEASE READ CAREFULLY BEFORE SIGNING RELEASE
I hereby acknowledge that I, the members of my family and our guests, in attending and using Splash Universe Water Park Resorts and related facilities located in Dundee, MI, do so at my, our, or their own risk. Splash Universe Water Park Resorts and their affiliated properties shall not be liable for any loss or damage to personal property or for any personal injuries sustained by me, members of my family or our guests in, on or about the premises. I assume full responsibility for any injuries or damages which may occur to me or members of my family or to our guests in, or about the premises. I do hereby fully and forever release and discharge Splash Universe Water Park Resorts, their employees and agents from any and all claims, demands, damages, resulting from or arising out of use of the facilities by me,the members of my family or our guests. I agree to hold Splash Universe Water Park Resort harmless from any and all claims which may be brought against them by my family members or guests. I hereby certify that I and the members participating in this function are in good health and are able to use the swimming pool and related facilities without assistance. I and the members participating in this function will not use the facilities under the influence of alcohol or drugs. I understand that no areas in the facility are designed for diving. I have read fully and agree to abide by the rules and regulations for the use of Splash Universe Water Park Resorts. (Read rules here) I represent that at the time of receiving and signing this agreement and release, I am of lawful age, legally competent and have authority to execute this release on behalf of my children and my guests. I further certify that before signing this agreement and release I have fully informed myself of its contents and I execute it with full knowledge thereof.
I understand that my minor child will be transported by HFM designated staff and/or volunteers OR a chartered bus service to and/from Splash Universe or Zap Zone.
I hereby give permission and release/discharge the Hemophilia Foundation of Michigan, its officers, agents, and employees from any and all claims or liability for personal injury or property damage that may arise from my child being transported to and/or from the SpringFest activities. In the event that my child is injured while being transported and requires the attention of a doctor, I consent to any reasonable medical treatment as deemed necessary by a licensed medical practitioner.
In the event treatment is called for, which a physician and/or hospital personnel refuse to administer without my consent, I hereby authorize the lead adult of the group to give such consent for us if I cannot be reached by telephone at one of the numbers on file or if, because of an emergency, there is not time or opportunity to make a telephone call. In the event it becomes necessary for that person to give consent for me, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care, should the cost of that care not be covered or reimbursed by the health insurance carrier.
