    Course Substitution Request

  • Students: Please complete all required items identified by the red asterisk (*).  In the Course to be Replaced field, please indicate the course that is currently on your degree audit that you need a substitution for. You also have the option of suggesting a course or requirement as a substitution in the Requested Replacement Course field. Please Note: Academic Departments make final determinations on substitutions.

    Also, please select the primary reason for the substitution.

    Graduation Issue refers to a situation where the course you originally needed is not scheduled to be offered again before you graduate. This should only be the case if you are within two semesters of graduating.

    Course No Longer Exists refers to a situation in which you are on an older degree plan that requires courses that are no longer being offered.

    You would choose Updating Program Catalog if you recently updated to a new catalog a need to substitute a course you are currently taking for a course on the new major.

    Finally, please indicate the person who recommended that you complete this form. We need this information to ensure that person knows that you submitted the form.

      Send to the Program Coordinator or Department Chair of the academic program the student is pursuing.

      Please review the request and complete the items below. The student may have suggested a course or requirement as a substitutionary course above, but the final decision is the Department's and should be entered in the Recommended Course Substitution field below.
