Replacement Diploma Request

This form is only intended for students who completed their degree with Southwestern Assemblies of God University (Waxahachie, TX) and are requesting a replacement diploma.

Degree and Major information are for clarification only. SAGU will verify the program information on file. Additionally, diplomas will have the signatures of the current administration. Except for the student name, program, conferral date, and administration signatures, the diploma is a finalized document and no other modifications will be made.

Only students or former students may request their diplomas. Requests from other persons or entities will be denied.

The cost for this service is $50. Please allow 2-4 weeks for processing and delivery. Diplomas shipped outside the U.S. may incur additional charges. SAGU does not produce electronic or digital versions of this diploma. Diploma is only available as a hard copy.

Students or former students who have not enrolled at SAGU in the past year need to submit a copy of their current driver's license.

Students who want a name listed on their diploma that is not the name that the University has on file will need to submit an SSN card, marriage certificate, or other legal documents with the new name.

SAGU does not permit the use of nicknames, aliases, or other monikers not derived from the student's legal name.
