  • Instruction for completeing the form. Please fill in the shift you worked each day; if appliacble. If you did not work please enter "N/A".

    Holiday: Enter Y for "Yes holiday hours worked" or N for "No holiday hours worked." Ex: If Jane Doe work from 7p-3a on July 3rd into July 4th. Jane would have 5 hours regular pay for the hours worked from 7p - midnight and 3 hours holiday pay for the hours worked from midnight - 3a on July 4th for the holiday. Remeber holidays are calculated from midnight (start of the holiday) to midnight (end of the holiday) and according to our holiday schedule.
