I am in the process of completing the health history questionnaire and I have a few questions regarding the section about previous surgeries. Could you please clarify if I need to include minor procedures or only major surgeries?
Additionally, I am unsure about how to categorize a surgery I had in my childhood. Any guidance you can provide would be greatly appreciated.
