Error message: disabled form

My form is enabled and yet it keeps showing this message. I can't preview or put on my website, although it showed up yesterday. How do I fix this? The form is the Tennis Invitational Entry form.

It is supposed to be under the Tennis tab on our website, for the Tennis Invitational

Your forms shows disabled because your account has been suspended due to violation of terms of use:

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
