How to have custom URL of my form

I would like to take the 'direct link of your form' and re-name it 'First Form ~ Student Information'; however, it looks as though I'll have to have the entire URL before that name.

http://www.jotformpro.com/Welvin/Web_Inquiry_Form (this form uses custom URL)

OR

If what you are after is to use your own domain name instead of jotform.com on the URL of your forms, then you will need to setup the custom domain name instead.
