How do I attach a pdf file to the autoresponder?

I have uploaded the pdf file in the advanced tab of the autoresponder email however when I send a test email the pdf is not attached. 

When you mention that you send a test email, do you mean you are sending the email using the "Test Email" button?

Please note that the test email will only test if you will receive the email properly. To properly test the content of your email, please do a test submission and see if you will receive the email properly with the proper content. I did a test submission on your form and I was able to receive the Autoresponder email properly with the attachments.
